Time Off Requests

Step 1: Submit the Time off Request
Step 2: You will receive an email confirming we got your request. (if you did not get an email contact the office immediately)
Step 3: Check the calendar by the next business day to see if your request was approved or denied

Immediately when you know of a schedule conflict that could interfere with working ours (8-5) submit the Time Off Request form and if approved it will show on the TEAM calendar by the end of business day. (Any requests submitted over the weekend will be evaluated Monday…with the exceptions of holidays)

We permit 12 absences per year. Each Absence is considered an occurrence.
Example: If you are sick for 1 day, that is an occurance and counted as 1 Absence. If you are sick for 4 days that counts as 1 occurrence and counted as 1 Absence and

All requests are first-come-first-serve. If you have requested a day that is already taken (which you will see on the calendar, your request will be denied, and encouraged to find another date.

We understand situations happen, and those will be marked as Absent From Duty. Any commitments that prevent you from us making schedule changes WITHOUT notifying us will be in violation of the schedule commitment made when you accepted the job.
If you are unavailable or turn away work due to prior commitments you will be marked Absent from Duty and required to fill out the Absent from Duty form in by 4pm that day as well as calling the office to notify us if you are available for work the next day.
Depending on the situation work cannot be guaranteed for the next day when Absent from Duty occurrence happens. Our schedule must resume with as little interruption as possible regardless.
More than 3 Absent from Duties in a 90 day period could result in a lighter schedule (less than 15 JTH). See Employee Handbook for more information

We strive for a HEALTHY workplace! We encourage everyone to maintain a healthy lifestyle, take vitamins during the flu season, etc. We require everyone to be fever free for 24 hrs before returning to work for the health of our staff and clients. If you are out more than 3 days from work due to illness, we will require a Dr note to return to work. Any illness will be marked as an Absent From Duty.

No. We will not be rescheduling clients upon request. Instead those hours will be reassigned to another technician. We ask the entire team to honor the schedule and work AROUND it. Not against it.
